End of Life Care supports clients in relieving the symptoms and stress that comes with terminal illnesses. At the end of life we want to give our clients the dignity that they deserve at the end of their life. End of life care involves a multidisciplinary team of health professionals, including nurses and consultants, to come up with a personalised approach of managing the symptoms of chronic illness. We also provide emotional support for families to allow them to cope during this difficult time.
End of life care offers dignity in this challenging time. We allow you and your family to make informed decisions about future medical treatment options and how you would like to be cared for. In this time we know it can be hard to deal with the death of a loved one, we work with the allocated NHS hospice teams to give you more peace of mind before and after your loved one passes away.
Services included:
Personal Care: Washing, dressing and grooming e.g. brushing hair, shaving, applying makeup, toileting and incontinence care, skin care (to prevent pressure sores) and mobility assistance.
Pain and symptom management: Making sure that your loved one is as comforted as possible by managing pain, nausea and other distressing symptoms.
Emotional And Spiritual Support: We address the emotional and spiritual needs of our clients. This may involve counselling, spiritual care and support groups to help everyone cope with the emotional challenges of the end of life process.
Family Support: We provide support to patients families and caregivers through this difficult and emotional time. This will involve counseling, respite care and assistance with practical matters.
Comfort and quality of life: We focus on improving your quality of life and providing opportunities for a heart-filled experiences, connection with loved ones and personal fulfillment during their remaining time.
